Optimizing Medication Management in Primary Care: A Medical-Social Approach Guided by Evidence-Based Policy
This seminar provides a comprehensive overview of medication management services in primary care and highlight evidence-informed practices in Australia. Professor Timothy Chen will share his experience and recommendations on evidence-informed policy changes including the integration of medication management services in primary care settings, government subsidization and service sustainability. Meanwhile, medical-social collaboration is also of uttermost importance in medication management services, social determinants of health (e.g. socioeconomic status, cultural beliefs and access to resources), strategies of medical-social collaboration and referral mechanisms for medication management services will also be shared in this seminar.
